Frozen shoulder (adhesive capsulitis) is a condition that results in the gradual loss of movement of the shoulder joint. It literally becomes “stuck,” and it can result in pain and restricted range of motion.

In a normal shoulder joint, the tissues within the joint (the capsule) expand and contract as the arm is moved. When these tissues become inflamed, scarring (adhesion) develops within the joint causing it to become difficult to move. The inflammation also results in vascularization–new and abnormal blood vessels that develop as part of the body’s attempt to heal the joint. Unfortunately, this allows a high amount of pro-inflammatory mediators into the joint, which worsens the condition.

Conservative treatment for frozen shoulder includes anti-inflammatory medications, steroid injections and physical therapy. If these are ineffective, surgery is often recommended. However, surgery carries risks along with a long and painful recovery, followed by intensive physical therapy.

This minimally invasive procedure is performed by the interventional radiologists at CU Medicine Interventional Radiology. Using a special catheter guided by x-ray imaging, the doctor “embolizes” or blocks the flow of blood through the abnormal blood vessels in the shoulder capsule. This reduces pain and inflammation, delivering significant relief within a few days.

Patients who have this procedure experience less pain and increased shoulder mobility, along with a reduced need for pain medication. Side effects are rare, and may include bruising or soreness in the area where the catheter is inserted into the bloodstream.
